100% A+ Care Contest

We want to reward you for making your health a priority. 
Below are the rules.  Have fun!

1. You must register to participate in this contest.  See Front Desk to register or click the link below.
2. You must pre-schedule (by 9/3) all of your adjustments and exams between 9/8 and 10/16, in accordance with your care plan.  (If you need to come more often than expected, that’s OK!  Those visits don’t have to be prescheduled.)  Minimum 6 appointments to qualify.
3. You must make ALL of your pre-scheduled appointments.  No missed appointments, no rescheduling.  This is what makes you an A+ Practice Member!
4. If you make all of your appointments, you earn a free adjustment ($65 value) simply because you have made your health a priority.
5. If you make all of your appointments, you will be entered into a raffle, where one lucky practice member will win the grand prize – choice of iwatch, fitbit, or athletic gear (yoga/cycling/running, etc.) – $200 value!
